Outreach care provides assessment programmes focusing predominantly on outdoor activities as a means of positively impacting on cycles of negative behaviour. Alongside the stability and structure of our 12-week programme, our young people are supported to complete a weekly planner to enhance time management skills and ensure their days are fully occupied. Throughout this process, we encourage them to experience as many different activities as they feel able to.
The home provides shorter breaks and bespoke respite programmes built around the same principles as the full assessment package where young people can access interesting and challenging leisure and recreational activities.
Outreach Care aims to support at most 3 persons within a homely environment, enabling them to live as independently as possible.
Outreach Care is managed by a qualified mental health nurse and NVQ 5 Health Care Workers, who have a track record of supporting people with mild mental health challenges.
Set up in 2019 Outreach Care aims to grow steadily in the coming years. It is our belief that everyone has the right to an active and fulfilling life.
